To change your name on the HCPC Register, you'll need to submit a request on your online account. You'll be asked to upload evidence such as a marriage certificate or deed poll documentation. Once submitted, we'll review your request and update the Register if it's approved.

Please ensure that the name you submit is the same as your practising name.

How to submit a change of name request

Log in to your online account. Scroll down to 'My Details' and click the link. On the next page, click the 'Update Personal Details' button and select 'Name' from the dropdown. Enter your name, upload your evidence and click 'Save'. This will send the request to us for approval.

Please note, we do not accept change of name requests via email.
