  We hope you will help us by completing this form but please be aware the form is entirely voluntary and, if you choose not to complete it, this will not affect your application for registration. Please do not re-scale this form when printing.
  
  
• Returners to Practice pack
  You must download and complete these forms if your approved qualification is over 5 years old or you have not practised your profession in the last 2 years.
  
  
• Standards of Conduct, Performance and Ethics
  These are standards that you must agree to keep to in order to be registered.
  
  
• Standards of Proficiency
  These are the professional standards which you must meet in order to be registered.
  
  
• Standards of Continuing professional development
  These are the professional standards which you must meet in order to stay registered.
