Download application forms
On this page you can download a copy of the forms for a new profession.
Please note that these forms are not for individuals who wish to be registered with the HPC. Individual members of professions that we already regulate should see the Apply section for more information.
Both documents are available in pdf formats to view online. However, when you are preparing an applicatoin, you should contact us to obtain a hard copy of the Application Form. Your application information must be entered on an original of the form, and not a downloaded copy.
